The /use parts are for trinkets 1 and 2, you can blank those out.

How do you get around the fact that AP and Icy share a CD?

Long delay in reply, but in the case of shared cooldowns on a macro like this, it would hit whatever isn't on cooldown at the time. If you value one over the other, simply alter the order they are in.
